REQUIREMENT:
MUST BE A CERTIFIED MEDICAL ASSISTANT OR 3 YEARS EQUIVALENT EXPERIENCE IN MEDICAL ASSISTANT WORK.
CANDIDATE MUST BE ABLE TO TRAVEL TO SHERMAN TX TWICE A WEEK .
the ideal candidate will spend 3 days a week in Fort Worth and two days a week in Sherman for patient enrollment
MUST BE ABLE TO HANDLE LARGE VOLUME OF PHONE CALLIING EACH DAY
This position is responsible for providing BOTH Face to face & non-face-to-face services to Medicare beneficiaries who have multiple chronic conditions that are expected to last at least 12 months, or until the death of the patient. Collaboratively with the DFWPMA care team, patients/families, and outside contacts the Chronic Care Management Coordinator will facilitate quality health care services for the DFWPMA patient population. The Chronic Care Management Coordinator will serve as a patient advocate and navigator to support better health outcomes for practice's patients. In addition, this position will be responsible for promoting clear communication amongst patients, families and care team regarding patient care plans, medication management, and coordination of care. Work is heavily telephonic in the Sherman office. Candidate must be able to travel between Fort Worth , Frisco and Sherman at least 2 days a week.
Some of the job responsibilities includes:
- Provides at least 20 minutes per patient per month of phone or email chronic care management services.
- Provides personalized care coordination between patient, providers, pharmacies, specialists, hospitals, and testing services.
- Utilizes Aunt Bertha and other on-line resource tools, to identify and access social service needs for eligible patients.
- Assists patients with medication management.
- Documents care plans and monthly outreach in the electronic health record (EHR).
- Assists the Medical Director and Director of Care Coordination in following initiatives and other tasks.
What you need:
- Current MA or work related experience preferred.
- Will consider 2-3 years of healthcare experience as an Medical Assistant, Care Coordinator, Case Management, etc.
- Experience with case management or utilization management preferred.
- Ability to work with electronic health records (EHR).
- Must possess strong interpersonal skills, communication and the ability to work effectively with a diversity of internal and/or external contacts.
- Must be able to present a positive and enthusiastic demeanor while directly working with patients.
Job Type: Full-time
Pay: $18.00 - $27.00 per hour
Expected hours: 40 per week
