About Job

This position reports directly to the Supervisor of Diversion Services within the BH division. This position provides support for Diversion Services programs, including TCOOMMI, OCR, and Jail Diversion, on an administrative level based in a trauma-informed and person-driven care model. Will work under supervision and as part of multi-disciplinary support teams that includes LPHA’s, QMHP’s, and prescribers to assist individuals in services to move toward recovery. This position participates in data collection, entry, analysis, and reporting as directed by program supervisors and the Program Administrator. This position has full knowledge of program guidelines, including Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) and Outpatient Competency Restoration (OCR) programming, and tracks and monitors compliance with program and grant requirements. This position is responsible for TCOOMMI WebApp review, data entry, and referrals and initial intakes into Diversion Services programming including those received from TCOOMMI, Mental Health Court, and/or probation or parole offices. This position is responsible for working with all Access, Diversion, and Intensive Services programs to aid in coordinating step down care from justice-involved services into outpatient treatment. This position will serve as a liaison in working with outside agencies as needed. This position may provide case management services if needed.

This position exemplifies service excellence by providing administrative support that will aid in increasing an individual’s access to case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services with a trauma-informed and person-centered approach.

This position practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair.

Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, TCOOMMI and OCR contracts, grants, and Lifepath Systems policies and procedures. This position will work closely with the Adult Fidelity Managers to ensure that all gaps are reconciled.

This position builds positive relationships with community-facing agencies and always maintains professional relationships with all community stakeholders and partners. Professional conduct with Peers, Leadership and Support teams is expected at all times. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with improving access to social determinants of health.

Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners; to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve.

Specific Targets: Will review the Magistrate Jail Report daily for individuals identified to be enrolled in LPS services and inform or coordinate care with appropriate LPS providers. Will prepare applicable reports and attend specialized courts bi-monthly and provide relevant data for the courts for individuals participating in the specialized courts. Will complete applicable reports due to probation office for LPS enrolled individuals. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record. Documentation which meets fidelity standards and documented into the electronic health record within 2 business days at a rate of 90% or above. Ensures all measures identifies in the TCOOMMI Program Guidelines and Processes (PGP) (01.01, 01.02, 01.04, 01.05, 01.06, 01.07, 01.10, 01.12, 01.13, and 01.14) related to TCOOMMI WebApp are completed and entered as outlined with 100% compliance rate.

If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

  • Bachelor’s degree in psychology, counseling, social work or related field
  • Two (2) years of experience in the treatment of individuals with mental illness, substance use disorders, and/or criminal justice involvement. If a shorter time, the individual’s experience, and aptitude to learn and perform role quickly will be taken into consideration.
  • Experience in Texas Recovery and Resiliency (TRR) required
  • Knowledge of Trauma-Informed Care Practices required.
  • Must have reliable transportation with a valid driver’s license and auto insurance.
  • Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention and symptom management services to individuals with serious mental illness or serious emotional disturbance.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to write clearly and professionally on all services and internal communications.
  • Ability to think clearly and logically while dealing with potentially dangerous and/or emotional situations.
  • Ability to respond to individuals with patience, empathy and understanding.
  • Organized, detail-oriented, and computer-proficiency.
  • Ability to demonstrate strong multi-cultural competency.
