Job Summary
Our organization is currently in need of a counselor to join our team and provide older adult patients with in home counseling. MUST HAVE at a minimum LSW or LPC. The ideal candidate will be qualified through training, education and experience to offer guidance and advice to patients, family members and other care providers. A successful candidate will have excellent communication skills, a patient demeanor and the ability to establish a relationship of trust with patients. MUST HAVE LSW OR LPC AS A MINIMUM

Responsibilities:

• Provide in home counseling.
• Counsel older adults.
• Maintain professional licensure or certification.
• Complete paperwork in a timely manner.

Duties

• Conduct individual counseling sessions in the patients home to address mental health issues.
• Implement crisis intervention strategies to assist clients in distress.
• Utilize cognitive behavioral therapy techniques to promote positive behavioral changes.
• Provide patient care by assessing client needs and developing personalized treatment plans.
• Collaborate with multidisciplinary teams, including social workers from the referring Area Office on Aging, to ensure comprehensive care.
• Maintain accurate documentation of client progress and treatment outcomes.

Requirements

• Proven experience in mental health counseling or related fields.
• Experience in individual counseling settings.
• Knowledge of patient care principles and behavioral health practices.
• Background in social work or counseling required.
• Ability to work effectively with older adults.
• Excellent communication skills, both verbal and written, for effective client interaction.
• A degree in social work, or counseling is required; relevant licensure required.

Job Type: Part-time

Pay: $30.00 per hour

Expected hours: 1 – 20 per week
