- Assists with the responsibilities and functions delineated in the discharge plan.
- Ensures that the administrative and clerical functions related to the admission and/or discharge of
residents is carried out to completion by coordinating efforts with the Healthcare Team Members.
- Maintains appropriate contact with hospitals, doctors and outside agencies to facilitate admissions.
- Coordinates all forms needed for admissions.
- Maintains state required resident roster.
- Coordinates financial updates for private pay residents.
- Completes appropriate section of the MDS.
- Participate and attend all in-service training and education programs as scheduled.
- Other duties as assigned.

Physical Demands, Work Environment, and Expected Hours of Work: The physical demands described here are

- This position is very active and requires frequent standing, walking, bending, kneeling, stooping and
climbing.
- This position will be required to lift or carry weight up to 50 lbs. and ability to push up to 250 pounds
independently.
- The individual must use proper body mechanics to assist residents in their daily living.
- This position regularly requires long hours and frequent night and weekend work.
No travel is expected although occasional travel may be required for training sessions, continuing
education opportunities, emergency situations, and other company functions.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- High school graduation or GED required.
- Bachelor’s degree in Social Work, or closely related human services field required
- 1-2 years related experience in a Healthcare setting.
- Two years’ experience in a similar social work position required.
- Two years of long-term care or rehabilitation experience desired.
- Ability to read, write, speak and understand English fluently.
- Ability to meet or exceed the company’s attendance and punctuality standards.
- Ability to use Electronic Records and miscellaneous software and office equipment.
- Ability to understand and follow directions as given.
- Ability to work with minimal supervision.
