The duties for the Mental Health Counselor II will include, but not be limited to the *****
Provides treatment team to develop an individualized plan for each family goals and needs.
Counseling families guiding to obtain services within the community in order to strengthen family relations.
Support family with services in their home, at school sites and agency offices where needed.
Provides transportation if required. Responsible for progress notes, provides crisis intervention as needed.
Provide Individual and Group Rehabilitation Services to clients in collaboration with the primary therapist, including, but not limited to, training in social skills, independent living skills, and pre-vocational skills. Services to be provided in the clinic, at assigned schools, and in the field (including homes and other community settings).
Link families with community resources and other community providers of services, (i.e., DCFS, probation, etc.).
Maintain ongoing communication with school contacts and participate in presentation of Youth Contact Program services to schools.
Conduct Case Management Needs Assessments during the initial intake process.
Maintain required minimum productivity requirements consistently.
Complete all documentation in a timely and comprehensive manner and by required deadlines in compliance with all DMH and Center requirements.
