About the Role:

We are seeking a compassionate, self-motivated, and experienced Regional Support Social Worker to join our dynamic healthcare team. This hybrid position plays a key role in supporting multiple skilled nursing facilities across New York and New Jersey, offering both on-site assistance and remote support as needed. The ideal candidate will bring strong interpersonal skills, a passion for geriatric care, and a flexible mindset.

Key Responsibilities:

• Provide short-term, on-site social work coverage to nursing facilities across NY and NJ, as assigned.
• Support social work departments with compliance audits, staff training, and survey preparedness.
• Conduct virtual follow-ups and remote documentation support when off-site.
• Offer educational coaching and mentorship to new or transitioning facility-level social workers.
• Maintain flexibility to travel several days per week to different locations, with scheduled work-from-home days built in.

Qualifications:

• MSW or BSW required (MSW preferred).
• Minimum of 2 years' experience in long-term care or skilled nursing facility setting preferred.
• Ability to work both independently and collaboratively in fast-paced environments.
• Reliable transportation and willingness to travel regularly between facilities.
