Medical Social Worker (MSW)
Location: Snellville, GA & Bozeman, MT

Position Overview
The Medical Social Worker (MSW) is responsible for delivering skilled social work services to clients in the home setting. This includes conducting assessments, providing counseling, assisting with care planning, and helping clients and their families address the emotional, psychological, and social challenges associated with illness. The MSW serves as an integral part of the interdisciplinary team, ensuring each client’s care plan is person-centered and supported by community resources when appropriate.
Qualifications
Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education
Current licensure to practice social work in the state of Georgia
Minimum of 1 year of clinical social work experience in a healthcare setting (home health, hospice, hospital, etc.)
Strong communication, organizational, interpersonal, and analytical skills
Essential Responsibilities
Provide therapeutic support to clients experiencing emotional or social issues affecting their health*
Assist clients and families in adjusting to illness-related stressors and changes in lifestyle or functioning*
Evaluate clients’ psychosocial needs through ongoing assessments and maintain current documentation*
Participate in the development and regular re-evaluation of individualized care plans (POC)*
Deliver counseling and education to clients and caregivers regarding coping strategies and available support*
Identify and coordinate appropriate community resources for clients in need*
Collaborate with physicians, nurses, and other care team members in care planning and service delivery*
Contribute to discharge planning and transitions of care*
Attend interdisciplinary meetings, staff training, in-services, and utilization reviews*
Supervise and co-sign documentation for Bachelor’s-level social workers as applicable*
Ensure documentation complies with care plans and agency policies*

Work Environment & Physical Requirements
Physical Elements:
Clear communication skills, both verbal and written, for client interaction and documentation
Visual ability to read and review care plans, records, and electronic files
Manual dexterity to operate phones, computers, and other office or field-based tools
Mobility to lift or move equipment and materials (up to 25 lbs) and to travel to patient homes
Work Setting:
Combination of office-based work and field visits to client homes
Exposure to a variety of residential environments, which may include pets, temperature variations, or limited space
Occasional interactions with emotionally distressed individuals or families
