Job Summary
We are seeking a PRN Case Manager (RN or LPN) to perform initial client assessments and conduct supervisory visits as needed. This is a flexible, per-visit position ideal for a nurse who wants to support quality home care in their local community without full-time hours.

Responsibilities Key Responsibilities:

• Conduct initial in-home assessments for new clients.
• Develop and update individualized care plans.
• Perform periodic supervisory visits to ensure quality of care.
• Monitor and evaluate caregiver performance and client satisfaction.
• Document visits in compliance with agency policies and state regulations.
• Communicate effectively with agency leadership regarding client status and staff performance.
• Ensure services align with agency standards and client needs.

Skills

• In-depth knowledge of physiology and medical terminology relevant to case management.
• Experience with medical records management and documentation practices.
• Familiarity with pediatrics and the unique needs of children in a healthcare setting.
• Proficiency in discharge planning processes to ensure smooth transitions from hospital to home or other care settings.
• Understanding of utilization management principles within managed care environments.
• Ability to work effectively in high-pressure situations typical of emergency medicine settings.
• Skills in addiction counseling to support patients dealing with substance use issues.
• Strong patient assessment skills to evaluate individual needs accurately.
