Social Worker Key Responsibilities:

• Conduct comprehensive psychosocial assessments to identify residents' needs and develop care plans.

• Provide individual and group counseling to residents and families to address emotional, behavioral, and adjustment issues.

• Coordinate with interdisciplinary team members to develop and implement resident care plans.

• Assist residents and families in accessing community resources, financial assistance, and support services.

• Advocate for residents' rights and ensure their preferences are respected in care planning and decision-making.

• Facilitate resident activities and programs to promote social interaction and engagement.

• Document assessments, interventions, and outcomes accurately in resident records.

Social Worker Qualifications:

• Master's degree in Social Work (MSW) from an accredited program.

• Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of Pennsylvania.

• Previous experience in a healthcare or nursing home setting preferred.

• Knowledge of geriatric social work principles, mental health issues, and community resources.

• Strong communication, interpersonal, and advocacy skills.

• Ability to work effectively within a multidisciplinary team and independently.
