Job Description:
Under the clinical direction of the Associate Medical Director, provides timely and complete screening, brief intervention, and psychosocial assessment to patients with or at risk for a substance use disorder (SUD) or of complications of substance use located within an Acute Care setting [Emergency Department (ED), Inpatient (IP), Birthing Unit, Bridge or Ambulatory clinic] prioritizing perinatal patients in these areas
- Services as the facility's SUD Women's Health Ambassador as part of the Addiction Consult team and the patient's ambulatory OB care team, conferring and consulting with D/ IP/ OP clinical service leads/ providers to provide a multidisciplinary approach to SUD patient care
- Assesses and identifies the patient's level of care, making recommendations of referrals, referring to a higher level of care as needed (inpatient detox or rehab)
- Completes intakes for H+H MSOP and OTP as necessary, coordinating follow-up services
- Conducts patient treatment/ discharge/ aftercare planning as it relates to SUD in collaboration with care team.
Minimum Qualifications:
Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS “Supporting Recovery with Medications for Addiction Treatment (MAT)” training.
For Assignment to Level II
1. Valid CASAC - Level 1 Certificate issued by OASAS; and one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or
2. Is otherwise considered a QHP, as described in “2” of Qualification Requirements for Level I above, and has one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or
3. Satisfactory equivalent of education, training, and experience. However, all must have a CASAC Level 1 Certificate issued by OASAS or meet the definition of a QHP as described in “2” above.
