Resident Care Manager/MDS Coordinator

Compensation:

• Retention Bonus: $15,000

• RN: $51.00 to $55.00 per hour (DOE)

• LPN: $47.00 to $52.00 per hour (DOE)

• Student loan forgiveness

• Preference for RNs, but LPNs will be considered.

Job Summary: The Dalles Health and Rehabilitation Center is seeking a dedicated and compassionate Resident Care Manager/MDS Coordinator to oversee the care of 22 residents, including both skilled and long-term care populations.

Essential Functions:

• Oversee daily care of assigned residents.

• Manage, create, and maintain Care Plans in compliance with CMS regulations.

• Conduct resident assessments and oversee MDS documentation to ensure regulatory compliance.

• Track, report, and evaluate residents for Medicaid Complex Criteria.

• Collaborate with interdisciplinary teams to optimize resident care and ensure accurate reporting.

• Manage the MDS schedule, ensuring timely completion and successful submission.

• Handle Certs and Recerts, ensuring compliance with CMS requirements.

• Provide mentorship and support to nurses and caregivers.

• Work closely with the Director of Nursing (DNS) to implement quality improvement initiatives.

Education, Licensure, and Experience:

• Graduate of an accredited school of nursing (Required).

• Current LPN or RN licensure in the state of employment (Required).

• Demonstrated knowledge of nursing processes, particularly related to geriatric residents, long-term care, and rehabilitation.
