Role Overview:

As a Discharge Planner (Home Health Transitions), you will coordinate post-hospital care by arranging in-home services, equipment, and follow-up appointments. Your thorough planning and communication ensure seniors experience a seamless discharge, reducing readmissions and improving outcomes.

Key Responsibilities:

  • Work with hospital staff, insurers, and families to create comprehensive discharge plans.
  • Arrange home health services, medical equipment, and medications.
  • Schedule follow-up appointments and coordinate transportation as needed.
