Compensation Range: $65,000 - $75,000 yearly commensurate with experience.

Overview:

The Licensed Social Worker will provide mental health, substance use, supportive counseling, crisis interventions, buprenorphine treatment and case management services to clients within our Harm Reduction and Syringe Exchange Programs. This worker also provides health education and care coordination support for Low Threshold Buprenorphine program participants as needed.

This provider must be familiar with Harm Reduction principles and interventions. This is a unique opportunity for licensed providers to focus on a humanistic approach to marginalized communities’ mental health needs following the harm reduction model. The ideal candidate will be familiar with evidence-based interventions and comfortable using data to guide practice. The LSW counselor reports to the Managing Director for Harm Reduction Services.

Responsibilities:

Primary:

• The social worker will receive mental health referrals for services from intake assessments, harm reduction counselor, or other designated program staff.
• Most participants will be seen on a drop-in basis allowing participants to get acquainted with behavioral health services and schedule time as needed.
• Will provide mental health to Buprenorphine participants as needed.
• Will provide individual and group MAT education and supportive counseling to individuals contemplating Buprenorphine treatment.
• Provides crisis intervention services and, if necessary, refers clients for psychiatric evaluations and other emergency services.
• Utilize Psychoeducation and Evidence Based approaches in providing patient education, medication adherence to prescribed anti-depressants, to facilitate treatment change, while referring clients to mental health services as deemed appropriate.
• Submit monthly and quarterly reports on a timely basis as requested by NYC and NYS stakeholders.
• Participate in meetings as scheduled by the NYC and NYS funding liaisons.
• The LSW tracks depression and/or anxiety or palliative care service outcome through conducting weekly depression screening via the application of the PHQ9 and/or GAD-7.
• Communicates and routinely coordinates with the Primary Care Provider to inform the Provider’s stepped-care process and support relapse prevention once patient has improved.
• Participate in morning clinic huddle meetings, case conference, and quality assurance/improvement processes.
• This licensed staff will be required to update or review pertinent policies and procedures as needed.

Secondary:

• Participates in case conferences to advance the ongoing coordination of clients’ care.
• Generates timely, accurate, and content substantive progress notes with treatment goals and interventions that are congruent with the clients’ diagnosis and consistent with the scope of the Coordinated Care Model.
• Generates timely, accurate encounter forms with diagnoses and appropriate CPT Codes.
• Partakes of one-hour program and/or clinical supervision. This provider will receive clinical supervision with LCSW as needed.
• Attends all training, supervisions, staff meetings and care planning sessions or as assigned by supervisor.
• Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
• Participate in quality assurance and quality improvement planning projects and activities.
• Participates in monthly peer workers development meetings and interventions as needed.

Qualifications:

• A master’s degree in social work or similar behavioral health discipline.
• A minimum of two (2) years of satisfactory full-time practice.
• A minimum of 1 year of Harm Reduction services experience preferable or working with marginalized populations.
• Current New York State Social Work License or related disciplines.
• Significant knowledge of health-related issues faced by injection drug users, including awareness of injection drug user networks, norms and culture.
• Culturally aware and sensitive to PWUD (People Who Use Drugs), PLWHA (People Living with HIV/AIDS), LGBTQAI, YIDU (Young Intravenous Drug Users), SMI (Severe Mental Illness), and MSM (Men who have Sex with Men).
• Proficient in Microsoft Office including Word, Outlook calendaring, PowerPoint, Excel and electronic data collection systems (AIRS, e-Share, EiCare, ECW).
• Have strong organizational, interpersonal, verbal, and written communication skills.
• Experience conducting and facilitating educational training preferred.
• Self-directed, motivated and able to work independently and as part of a team.
• Preferable training in the following areas:

  • Viral Hepatitis 101
  • Motivational Interviewing
  • LGBT Sensitivity
  • Syringe Exchange and Harm Reduction Training
  • HIV confidentiality and risk-reduction counseling
  • Overdose Prevention Strategies

Physical Requirements:

• Frequently remaining in a stationary position, often standing or sitting for prolonged periods.
• Occasionally moving around to additional worksites.
• Occasionally lifting and moving objects up to 25 pounds in ordinary office-related tasks.
