About the position:

The mission of the Americares Free Clinics is to provide quality health care to low-income, uninsured patients in a setting where all individuals are treated with dignity and respect. Americares opened its first clinic in 1994 to expand access to care for the working poor. Over the years, our program has grown to become the largest free clinic network in the state with four locations serving 2,000 patients annually. Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.

The Patient Care Coordinator will be the first person that patients interact with upon their first visit and every visit after. The role involves ensuring the patient meets criteria to be a patient at the free clinic, scheduling appointments, interpreting when needed and keeping the flow of patients in and out of the clinic on a day-to-day basis.

Key Outcomes:

In the first 90 days, the PCC will:

  1. Learn and live the Americares values.
  2. Complete all required new hire onboarding trainings.
  3. Demonstrate an understanding of Athena, the EMR used to maintain accurate and complete records.
  4. Demonstrate an understanding of the process associated with onboarding new patients, scheduling, and rescheduling of patients.
  5. Demonstrate an understanding of the daily flow of patients in and out of the clinic.

In the first 6 months, the PCC will:

  1. Demonstrate full competency with Athena.
  2. Schedule and reschedule patients independently and with accuracy.
  3. Manage the onboarding process for new patients with minimal assistance.

In the first year, the PCC will:

  1. Demonstrate success in onboarding, scheduling, and rescheduling patients independently and with efficiency.
  2. Demonstrate full understanding of the flow of the clinic.
  3. Demonstrate understanding of the resources available to patients outside of the clinic and ability to coordinate those resources.
  4. Coordinate and provide patient records to the referral source and provide patient follow-up.

Duties and Responsibilities:

  1. Actively model and contribute to Americares values, work culture and mission.

  3. Conduct initial and annual screening process to obtain proof of income and residency to determine eligibility for clinic services.
  4. Register patients into Athena once determined to be eligible; update information as needed.
  5. Conduct intake and outflow activities during clinic sessions.
  6. Schedule patient appointments both on and off the clinic site.
  7. Provide patient records to the referral source and advise the patient of the details of the follow-up visit.
  8. Other duties and responsibilities as assigned.

Degrees and/or experience required for the role:

High school graduate or equivalent.

Prior experience with front desk services in a medical, surgical, or clinical setting.

These competencies are required for the role:

  1. Professional fluency in Spanish.
  2. Demonstrated ability to work with a diverse team, including medical staff, volunteers and patients.
  3. Demonstrated ability to manage multiple priorities while maintaining a high level of attention to detail and accuracy.
  4. Demonstrated experience providing positive customer service and experiences to patients.
  5. Demonstrated familiarity with medical terminology.
  6. Demonstrated proficiency with an electronic medical record.

US work authorization is required for this role.

  1. Prior experience in a free clinic.
