Job Summary
We are seeking a compassionate, organized, and detail-oriented Social Worker to join our team supporting individuals with developmental disabilities. The Social Worker will play a key role in developing and implementing individualized service plans, coordinating admissions and discharges, and serving as a primary liaison between the organization, individuals served, and their families or guardians.
Duties
- Service Planning & Coordination: Develop, implement, and monitor Individualized Service Plans (ISPs) in accordance with state and agency guidelines. Conduct assessments and gather input from interdisciplinary teams, individuals served, and their families to ensure plans reflect current needs and goals. Track progress, update plans regularly, and ensure compliance with timelines and regulatory standards.
- Admissions & Discharges: Coordinate the admission process including intake assessments, documentation, and orientation for new individuals and families. Facilitate smooth discharge planning, including coordination with external providers and ensuring aftercare plans are in place. Maintain accurate and up-to-date case records and documentation.
- Communication & Family Engagement: Act as the primary point of contact for families, guardians and other stakeholders. Facilitate regular updates, meetings, and communications with families regarding the individual's care and progress. Address concerns, questions, and feedback with professionalism and sensitivity.
- Collaboration & Advocacy: Work collaboratively with direct support staff, healthcare professionals, case managers, and other team members. Advocate for individuals' needs within the care team and community. Participate in multidisciplinary team meetings and contribute to holistic care planning. Contributes to team effort by accomplishing related results as needed. As assigned, provide support to the President with various duties and assignments. Any other duties as assigned.
Qualifications
- Knowledge of developmental disabilities and related services
- Strong interpersonal and communication skills
- Ability to work independently and as a part of a team
- Cultural sensitivity and ethical practice
- Understanding of relevant laws and regulations
- Strong understanding of person-centered planning and developmental disability services
- Excellent communication, organizational, and interpersonal skills
- Ability to manage multiple tasks and prioritize effectively in a dynamic environment
- Proficiency in documentation and case management software is a plus

Job Type: Full-time
Pay: $45,000.00 - $52,000.00 per year
