Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in behavioral health and experience in providing patient care in various settings, including hospitals and home community care. This role involves assessing patient needs, developing treatment plans, and facilitating group therapy sessions to support individuals with diverse challenges, including those with disabilities.

Responsibilities

• Conduct thorough patient assessments to identify needs and develop individualized care plans.
• Provide psychotherapy and counseling services to patients, utilizing evidence-based practices.
• Facilitate group therapy sessions to promote peer support and shared experiences among patients.
• Collaborate with healthcare professionals to coordinate comprehensive patient care.
• Maintain accurate medical records and documentation of patient interactions and treatment progress.
• Advocate for patients' rights and access to necessary resources within the community.
• Support individuals with disabilities by implementing tailored strategies for their unique needs.
• Stay informed about current trends in psychopharmacology to assist patients effectively.

Qualifications

• Bachelor’s or Master’s degree in Social Work or a related field from an accredited institution.
• Experience in behavioral health settings is preferred, particularly in group therapy and patient assessment.
• Familiarity with medical records management and documentation practices.
• Strong interpersonal skills with the ability to build rapport with patients from diverse backgrounds.
• Knowledge of psychopharmacology principles is a plus.
• Ability to work collaboratively within a multidisciplinary team environment.
