Housing Case Manager

Job Summary:
We are seeking a highly motivated, experienced individual to join our Handy Help Team. The Housing Case Manager provides professional-level service coordination and case management for individuals who are experiencing housing insecurity and moving into a Handy Help associate apartment or into the community of the client’s choice.
As a Housing Case Manager, you will assist clients who have experienced homelessness from initial entry into our housing program.
Through direct and collaborative assessments, you will create an individualized plan to identify barriers that have kept clients from finding long-term housing and help resolve those barriers.
Our team at Handy Help has years of experience supporting clients who often are living with physical or mental health diagnosis such as substance abuse, mental illness, traumatic brain injury and other co-occurring disorders.
The HSS will help our clients achieve their personal goals and maintain housing stability such as procuring vital public benefits. Our work environment requires critical thinking combined with the ability to hold compassion and accountability for self and others.

This approach makes our services easily accessible, particularly for clients with limited English proficiency who may prefer to engage with a mental health professional sharing a similar cultural background, instead of relying heavily on interpreters.

Position Requirements:
Education and Experience:
- Provide lease education, tenant handbook education and other pertinent education for the client.
- Work collaboratively with the on-site Support Services Manager as an advocate and intermediary between the client and the proper manager if/when lease violations or other housing problems occur.
- Create Eviction Prevention Plans with client and assist client in meeting goals of Plan.
- Provide direct support when appropriate, such as hoarding management and organizing in unit.
- Provide transportation and/or accompaniment for client to housing-related appointments (eg, furniture, lease signing, GRH benefits procurement, etc.). Accompany client to medical/dental and/or mental/chemical health appointments as appropriate.
- Provide assistance with grocery shopping, cooking, budgeting skills, and other independent living skills as needed to ensure success once in housing.
- Understand and uphold the Vulnerable Adults Act and HIPPA regulations.
IV. Position Requirements:
Education and Experience:
High School Diploma (Or equivalent)
- Bachelor's degree in human services or related field (Preferred)
Knowledge, Skills and Abilities:
- Have a sensitivity and aptitude for working with individuals who were formally homeless and persons living with chronic mental illness and chemical health problems.
- Ability to work respectfully and communicate effectively with multi-cultural, diverse populations in what can be at times stressful and emotional situations.
- Ability to work with an interdisciplinary team.
- Ability to work independently and collaboratively, exercise good judgment and pay close attention to detail.
- Strong organizational and problem solving skills, and the ability to multi-task.
- Ability to develop support-based relationships with clients who are coming out of histories of homelessness and/or housing instability.
- Excellent interpersonal communication skills; verbal and written.
- Ability to maintain professional boundaries and practice self-care in a position that is confronted often with physical illness, mental illness and chemical abuse issues.
- Must have the emotional stability needed to cope with the mental and emotional stress of the position.
- Ability to maintain confidentiality of client information.
- Access to reliable vehicle and ability to maintain good driving record and insurance levels, as directed by the agency, to fulfill job duties that require travel into the community.
Licenses and/or Certifications:
- Driver’s License, State ID or Passport

Pay: $50,000.00 - $60,000.00 per year
