Job Summary
We are seeking a dedicated and compassionate individual to join our Revere Home Care team. The Care Manager of Home Care Services plays a vital leadership role in overseeing the delivery of home care services. This individual will be responsible for supervising staff, coordinating client care, maintaining compliance with Washington State regulations, and promoting excellence in service delivery. This role blends administrative oversight with compassionate client care management—making it ideal for a leader who thrives in a mission-driven environment.
Responsibilities
- Assess client needs and develop individualized care plans to meet those needs effectively.
- Monitor client progress and adjust care plans as necessary based on evolving health conditions and personal care needs.
- Serve as a primary point of contact for the clients and their families, providing education, resources and support.
- Facilitate communication between clients, families, and healthcare providers to enhance understanding of medical conditions and treatments.
- Document all interactions and updates in the client's records accurately to ensure compliance with all home care standards.
- Advocate for the clients’ needs and well-being.
- Oversee recruitment, hiring, orientation, and ongoing professional development of staff.
- Coordinate with the scheduler to assign and monitor caregiver shifts and coverage.
- Conduct initial client assessments and periodic reassessments, and develop individualized care plans.
- Ensure compliance with all local, state, and agency regulations and policies.
- Perform annual performance evaluations and in-field supervision visits.
- Oversee training efforts to ensure caregivers are informed and compliant.
Skills
- Experience working with KPIs in a performance-driven environment.
- Familiarity with caregiver scheduling systems and EMRs.
- Ability to manage competing priorities in a fast-paced setting.
- Marketing and outreach experience.
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team setting.
- Strong organizational skills with attention to detail in managing patient documentation and care plans.

Pay: $65,000.00 - $75,000.00 per year
