JOB SUMMARY

The Resident Care Coordinator:

  • Participates with hiring, training, and scheduling of clinical team members.
  • Coordinates all services relating to resident care needs.
  • Monitors resident care needs.
  • Alerts the DHW and DOV when the resident has a change of condition.
  • Audits the medication administration program to ensure accuracy and completeness.
  • Addresses resident and family concerns during work hours.
  • Responds to resident special requests.
  • Acts as a liaison with community providers.

DUTIES AND RESPONSIBILITIES

  • Utilizing On Shift; set schedules for clinical team members, post no later than the 4th Monday of each month and oversee coverage of all shifts. Check that no overtime is scheduled, and schedules are being kept up to date in a manner required by the state. Maintain monthly filing of schedules in a community survey readiness binder.
  • Responsible to participate in coordinating onboarding, training and orientation for all new Care Managers and Medication Managers.
  • Audit Relias training completion, complete skills checklist and ensure onboarding compliance and completion.
  • Coordinate interviews for staff level employees. Including initial screens, sourcing and managing Applicant Tracking System (ATS), and scheduling interviews with hiring managers.
  • Provide feedback to/notify Executive Director, DHW and/or DOV any non-compliance to care with clinical team members.
  • Attend daily standup meetings, reporting any resident issues as documented in the Communication Log, Alert Charting Log, or Incident Reports.
  • Monitor resident care needs, alert the DHW and/or DOV or resident’s health care provider when the resident has a change of medical condition.
  • Review all medication orders daily and follow up non-preauthorization’s.
  • Attend weekly Service Plan meetings. Ensure input from caregiving team members and complete Service Plan review notes in Yardi and the resident’s medical chart.
  • Check the Service Plans regularly for specific resident’s needs. Assure follow through with the services as described in the Service Plan. Report any discrepancies to the DHW and/or DOV.
  • As directed by physician orders and DHW and/or DOV dispense and order medications as needed by residents for whom we provide medication administration services, where permitted.
  • Monitor all ordered supplies.
  • Coordinate resident appointments when required.
  • Review all physician orders for correctness. Assure resident’s physician orders are reviewed and signed periodically.
  • Audit the EHR Medication Administration Record weekly. Document findings providing a copy to the DHW and/or DOV.
  • Assist with the new resident move in process. Assure initial resident paperwork and physician orders are on hand. Communicate to the caregiving staff the new resident’s needs and preferences. Assure the kitchen staff has been informed of food allergies or special dietary needs prior to resident’s arrival for the first mealtime.
  • On call duties on a rotating basis on the weekends and routinely on the weekdays. While on call, responsible to triage emergency care needs, assure staffing needs are met, and respond to other miscellaneous problems that may arise.
  • Perform Quality Assurance Audits as directed and provide feedback/results to DHW, DOV, and ED, as necessary.

BUDGETING

  • Responsible to monitoring employee hours worked and staffing patterns to assure the community remains within projected budget. Weekly overtime will be closely monitored by leadership.
  • Responsible to monitor weekly the declining balance sheets for all departments to assure the community remains within projected budget.

QUALIFICATIONS / EXPERIENCE

  • Recent history of experience in assisted living and memory care setting.
  • Recent history of medication manager or care manager experience in assisted living and memory care setting.
  • Excellent customer service and communication skills.
  • Ability to build trust and relationships with leadership, as well as with clinical/care staff.
  • Proven organizational skills and sense of urgency.
  • Comfortable making decisions and directing others, including those who are not direct reports (this is not a managerial role).
  • Able to perform frequent lifting of less than, or equal to, 50 pounds.
  • Able to occasionally lift more than 50 pounds.
