This position supports the outpatient Psychiatry department at Takoma Theatre, providing care coordination for patients with complex psychosocial challenges. The social worker will facilitate interventions, support discharge planning, and connect patients with community resources. Knowledge of DMV community services is preferred.

Minimum Education: Master's Degree in Social Work (Required)

Minimum Work Experience: 3 years post-MSWS experience (Required)

Required Skills/Knowledge: Excellent communication, cultural sensitivity, ability to relate to diverse patient populations.

Required Licenses and Certifications: Licensed in DC at LICSW level; MD and/or VA licensure if applicable.

Responsibilities include:

• Developing, implementing, and documenting patient/family care plans.
• Facilitating multidisciplinary communication and discharge planning.
• Utilizing external resources to meet patient needs.
• Addressing complex barriers to care and mediating conflicts.
• Providing clinical leadership, mentorship, and staff education.
