Job Summary
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will have experience in retirement home settings and a strong understanding of care plans, home care, and long-term care services. This role is essential in ensuring that residents receive the highest quality of care, particularly in memory care and dementia care environments. The Care Coordinator will work closely with families, healthcare providers, and staff to manage individual care needs and promote the well-being of our residents.

Responsibilities

  • Develop and implement personalized care plans for residents based on their unique needs and preferences.
  • Coordinate with healthcare professionals, including nurses and social workers, to ensure comprehensive care delivery.
  • Manage daily operations related to resident care, including scheduling appointments and overseeing medication management.
  • Provide support to families by offering resources and guidance regarding dementia care and memory support.
  • Facilitate communication between residents, families, and staff to address any concerns or changes in health status.
  • Manage the staffing needs of the memory care Community by interviewing, hiring, training, supervising, and scheduling of caregiver and scheduling med tech coverage.
  • Monitor the effectiveness of care plans and make adjustments as necessary to enhance resident outcomes.
  • Conduct regular assessments of residents' physical, emotional, and social needs to ensure optimal quality of life.
  • Lead training sessions for staff on best practices in long-term care and community support.

Requirements

  • Previous experience in a retirement home or similar environment is preferred.
  • Knowledge of care plans, home community care services, and long-term care regulations is essential.
  • Background in nursing, social work, or related fields is highly desirable.
  • Experience with memory care and dementia care practices is a plus.
  • Strong organizational skills with the ability to manage multiple cases effectively.
  • Excellent communication skills for interacting with residents, families, and healthcare professionals.
  • Compassionate attitude towards caregiving with a focus on enhancing the quality of life for residents.
  • Ability to work collaboratively within a team-oriented environment.

Job Type: Full-time

Pay: $74,000.00 - $84,000.00 per year
