Luna Del Valle Healthcare Services is seeking a compassionate and experienced Medical Social Worker (MSW) to support patients and families across two of our specialized divisions: Indigo Sun Palliative Care and LDV Hospice. This is a full-time position (40 hours/week) with 20 hours dedicated to each division. Luna Del Valle Healthcare Services is seeking a compassionate and experienced Medical Social Worker (MSW) to support patients and families across two of our specialized divisions: Indigo Sun Palliative Care and LDV Hospice. Key Responsibilities:
Conduct thorough psychosocial assessments for patients and families.
- Develop and implement care plans addressing emotional, social, and practical needs.
- Provide supportive counseling to individuals coping with serious illness.
- Facilitate family meetings to support complex decision-making and communication.
- Guide advance care planning discussions and documentation.
- Connect patients with community resources such as financial assistance, transportation, home services, and support groups.
- Offer grief support and bereavement resources.
- Provide crisis intervention as needed.
- Advocate for patients within the healthcare and social service systems.
- Participate in interdisciplinary team meetings to ensure cohesive and coordinated care.
- Maintain timely and accurate documentation in accordance with regulatory and program standards.
- Contribute to program development and quality improvement initiatives.
Master’s Degree in Social Work (MSW) from an accredited program.
- Current state licensure required (LCSW preferred).
- At least 2 years of healthcare experience, preferably in palliative care, hospice, or chronic illness management.
- Current CPR certification.
Valid driver’s license and current auto liability insurance. - Strong knowledge of community resources, Medicare/Medicaid, and care planning tools.
- Compassionate approach with excellent communication, organizational, and interpersonal skills.
- Experience working with culturally diverse populations.
- Ability to work independently and collaboratively with a multidisciplinary team.
- Comfort discussing sensitive topics such as serious illness, dying, and grief.
$31.50 – $35.00/hour based on hospice/palliative care experience.
