Pay Rate: $34.50/hour

Park Avenue Extended Care in Long Beach, NY is seeking a Part-Time Social Worker to join our interdisciplinary team. This is an excellent opportunity to provide vital psychosocial support to residents and their families in a welcoming and supportive environment.

Key Responsibilities:

• Assess the needs of newly admitted residents and their families for treatment planning.

• Provide social work services addressing psychosocial issues related to illness or placement.

• Develop individualized care plans with appropriate treatment interventions.

• Participate in comprehensive care team meetings and represent resident/family concerns.

• Foster collaborative relationships across all departments to ensure quality care.

• Lead or participate in family meetings and therapeutic group sessions.

• Ensure compliance with federal/state regulations and documentation standards.

• Provide guidance on community resources to assist with discharge planning.

• Contribute to interdisciplinary education and training sessions.

• Act as a liaison between families and facility staff to resolve concerns effectively.

Qualifications:

• Master’s Degree in Social Work preferred

• Minimum of 1 year of experience in a medical or healthcare setting

• Spanish-speaking candidates strongly preferred

• Strong interpersonal and communication skills

• Knowledge of community resources and discharge planning processes
