Are you passionate about helping others and looking to be part of a team that feels like family? We are an in-home care agency dedicated to providing compassionate, quality care — and we’re looking for a Care Coordinator to join our team!

As a Care Coordinator, you’ll play a vital role in supporting both our clients and caregivers. Some of your key responsibilities will include:

  • Conducting client intakes, monitoring visits, and completing assessments
  • Ensuring documentation is accurate, clear, and confidential
  • Performing internal audits to uphold the highest standards of care
  • Guiding and training field caregivers to support their growth and success
  • Assisting the scheduler in matching clients with the right caregivers
  • Working closely with the RN and Administrator to ensure quality care
  • Verifying that care plans are followed correctly
  • Providing direct care in the field when needed
  • Creating and maintaining caregiver/client schedules to meet everyone's needs

  • Basic office and caregiving experience
  • Strong computer skills
  • Excellent communication and organizational abilities
  • A heart for caregiving and a team-first attitude

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
