SUMMARY
The registered nurse Care Manager plans, organizes and directs home care services and the communication with the ordering physician, patient and family members for the assigned case load. Provide quality patient care via the development, implementation & evaluation of individual patient care plans. Acts as a liaison between the Agency, patient, and family or caregivers. Follows established professional standards of care, Agency guideline policy and procedures as well as accreditation, federal, state and local laws and regulations regarding home health care. Ensures through clinical staff schedules and documentation that care is being provided based on physician orders, established plan of care, and the regulations that govern home health care.
MINIMUM REQUIREMENTS
- Registered Nurse currently licensed in Puerto Rico. Bachelor of Science degree in Nursing preferred.
- Minimum of three (3) years’ experience as a licensed, registered nurse, including home care clinical experience.
- Minimum of two (2) years’ experience in a supervisory position, preferred
- Experience in a quality assurance position in acute and/or home health, preferred.
