Advanced Home Health and Hospice of Colorado is looking for a dedicated and compassionate MSW with a heart for supporting patients and families. In this role, you will provide emotional support, resource coordination, and advocacy for hospice and home health patients and their families.

Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans.
• Provide emotional and grief support to patients and their families.
• Assist with advance care planning and resource procurement.
• Collaborate with the interdisciplinary hospice team or home health team to ensure comprehensive care.
• Assist with volunteer management, including recruiting, training and coordinating hospice volunteers.
• Maintain accurate and timely documentation.

Qualifications:

• Master’s degree in Social Work (MSW) required
• Experience in hospice, home health, palliative care, or medical social work preferred
• Strong interpersonal, communication, and advocacy skills
• Compassionate and patient-centered approach
• Excellent written and verbal communication skills

• Monday to Friday
• Weekends as needed

Experience:

• Hospice or Home Health: 1 year (Preferred)
