2 Positions Available

Job Purpose:

Under the direction of the Mobile Health Operations Manager, the Mobile Health Care Coordinator works as part of the Tapestry Health team to ensure high quality and low threshold access to harm reduction-oriented health care, treatment and linkage to care. They will use the Harm Reduction approach to provide direct clinical services, harm reduction care and other needed support services to individuals who engage in high-risk sexual and/or drug-using behaviors throughout Western MA.

Duties and Responsibilities:

Clinical Services

· Provide confidential counseling sessions with people in need of medical care and related clinical services both in person and through virtual platforms. Assist trained staff in providing appropriate medical care. This includes, but is not limited to wound care, medication for substance use disorder, gynecological problems, STIs, Hepatitis C, HIV/AIDS, contraceptive methods, emergency contraception, pregnancy testing and options, PrEP, and PEP.

· Provide clinic support services and office support, including the use of electronic medical record system, phone counseling, and appointment making.

· Perform limited medical and lab procedures including but not limited to blood draws, urine testing, and guidance with participant/patient self-collected specimen collection

· Prepare and submit lab specimens to appropriate laboratory per laboratory and agency requirements

· Assist in the maintenance of referral information for other medical services and linkage to care

· Assist with supported referrals to health services, mental health, substance use treatment, and other clinical services as needed

· Assist with clinical participant follow-up and tracking as per protocol when needed.

Harm Reduction Training, Education and Engagement

· Provide harm reduction supplies to participants including syringe access and distribution, safer use supplies, naloxone distribution and education, risk reduction education and substance use and sexual harm reduction counseling

· Provide education and support as it relates to supporting the immediate needs of people who use drugs/ engage in high-risk activities and their sexual partners.

· During all interventions, provide education and care as it relates to supporting the immediate needs of people who use drugs/ engage in high-risk activities and their sexual partners.

· Conduct outreach including tabling or presentations at various community sites, as assigned.

Outreach

· Collaborate with other internal and external outreach teams in joint outreach efforts in coordination with the Mobile Health Operations Manager

· Help to maintain cooperative relationships with key leaders that include community leaders, agency staff, external agencies, business owners and others within the community.

· Provide direct service at harm reduction and health services offices when needed in coordination with Mobile Health Operations manager

· Assist in coordination of participant transportation needs to and from Mobile Health van as appropriate

Administrative duties

· Maintain appropriate and timely electronic documentation via Tapestry and external reporting systems per requirements.

· Verify, document and submit billing information per agency requirements.

Other

· Operate/drive the Mobile Health van and other agency vehicles in accordance with federal/state law and agency policies to perform essential duties

· Regular travel to Tapestry sites, local service agencies and community locations. Approximately 20% of working hours will be dedicated to travel time.

· Attend trainings, service planning sessions and other activities to ensure the most up-to-date and culturally appropriate services can be provided

· Assist in training and support of staff, interns and/or volunteers

· Maintain the highest level of professionalism at all times

· Other related duties as assigned

Qualifications

· Commitment to working with diverse communities and underserved populations.

· Working understanding of harm reduction principles and practice.

· Strong working knowledge of substance use disorder and behavioral health.

· Strong interpersonal skills, including experience in dealing professionally with challenging participants.

· Ability to work as part of a team in non-traditional settings, including a mobile clinic and during street and other community outreach.

· Experience with blood draws or a willingness to be trained to draw blood.

· Strong written and oral communication skills.

· Valid Driver’s License and access to reliable transportation.

· Must be able to operate a mobile health van or company vans to perform the essential functions of the position, which includes transporting clients in company vehicles

· Supports the mission and goals of the agency.

· Bilingual (Spanish/English) preferred

· 2-year college degree in related field or combination of education and job or personal experience equivalent to a 2-year college degree preferred

· Experience working with people engaged in high- risk activities related to sexual and drug using behaviors preferred.

· Computer skills required, experience with an EMR preferred.

· Experience as an EMT B, Medical Assistant, or LPN preferred.

Working Conditions:

· Evening and weekend availability occasionally required.

· Position schedule dependent on needs of participants and programs.

· Potential exposure to blood borne pathogens.

· Direct exposure to shooting galleries, bars, clubs, and other areas where high risk sex and drug use regularly occur required.

· Ability to deal with stressful situations.

· Ability to be outdoors for long periods in all weather conditions.

· Ability to work and travel in close quarters, including on a mobile clinic van, for long periods.

Physical Requirements:

· Ability to operate a mobile health van.

· Ability to sit at a desk (2 hours/day) and conduct general administrative duties through computer and phone applications as well as stand/walk for periods of time (6 hours/day).

· Ability to lift and carry up to 50 pounds.

Job Type: Full-time

Pay: From $22.29 per hour
