Title: LTSS Care Manager
Pay: $27.00 per hour
Hours: Monday-Friday 8 am-5 pm
Location: Hybrid/Fieldwork - This role will require monthly and quarterly member contact and will include 80% travel around Lake Worth/West Palm Beach, FL.

Position Purpose: The LTSS Care Manager assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. The LTSS Care Manager may develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

Required Skills & Education:

• 2+ years Field case management experience, ideally with Geriatric long-term care patients. (team handles members who are 65+)
• Experience with electronic medical records
• Strong computer skills – MS office
• Bachelor’s Degree
• Valid Drivers License
• Bilingual in Spanish

Preferred:

• Long term care experience
• Home health
• Discharge planning
• Bilingual - English/Spanish
