To provide administrative support to assigned home care team by coordinating operational functions that are consistent with program expectations and standards that are expected to allow for streamlined processes that support regulatory compliance and improved care quality from that location.

Responsibilities
- Referral Collaboration: Collaborates with Intake Referral Specialist to ensure proper documentation is obtained for ALL referrals.
- Referral Collaboration: Understands home care intake/referral process to assist Intake Referral Specialist when needed.
- Referral Collaboration: Supports EveryStep customer service reputation by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts. This includes EveryStep staff, patients, families, and referring partners.
- Referral Collaboration: Establishes and maintains positive working relationship with current and potential referral sources.
- Scheduling Coordinator: Coordinates the schedules for home care aides to ensure schedules are maintained, updated, and communicated to field staff and managers, and notifies Clinical Manager and/or Team Director timely of inability to cover requests for service. Staff assignments are scheduled 2 weeks in advance of care.
- Scheduling Coordinator: Notifies the Clinical Manager and/or the Team Director of unresolved discrepancies between visits ordered, scheduled, authorized, documented, and follows-up to ensure resolution.
- Scheduling Coordinator: Implements and utilizes work methods and best practices to ensure clients are admitted in accordance with corporate policy and procedure.
- Scheduling Coordinator: Ensures compliance with state, federal and other referral/intake regulatory requirements. Actively protects HIPAA regulations by ensuring patient information is kept secured and private.
- Care Coordination Liaison: Serves as the primary point of contact during office hours for all patients/families/partners for connecting members of the care team and actively responding to meeting patient needs.
- Care Coordination Liaison: Collaborates with team to ensure back-up for telephone reception when unavailable for answering calls.
- Care Coordination Liaison: Responsible for obtaining physician signatures timely. Examples but not limited to include Face to Face and physician orders.
- Care Coordination Liaison: Responsible for reviewing care documentation to ensure information is complete and accurate as part of every patient's care record.
- Care Coordination Liaison: Completes admit and discharge audits timely and accurately for reporting findings to Clinical Manager, Team Director and/or QAPI manager.
- Care Coordination Liaison: Participates in quality improvement process as requested for ensuring effectiveness and efficiencies for team function and office operation. Actively seeks ways to improve workflows for supporting team members.
- Administrative Support: Serves as a primary resource for the team for office duties such as distribution of mail, copies, filing, and telephone reception.
- Administrative Support: Assists the Clinical Manager and/or Team Director with managing the supply closet and supply orders.
- Administrative Support: Provides general clerical support to the Clinical Manager and Team Director including assistance with weekly staff meeting, agenda, and minutes.
- Other duties: Engages in duties as assigned for supporting the function of the home care administrative office.
- Other duties: Actively participates in training and education as assigned.
- High school diploma or equivalent
- Healthcare experience preferred
- 2-year Medical Record degree, business practices or secretarial training preferred
- At least 2 years administrative assistant/office manager experience preferred
- Knowledge of electronic clinical record systems, preferably Allscripts
- Quality customer service skills, ability to maintain confidentiality,
- Organize and prioritize activities, use own initiative, good judgment, and dependability
- Ability to communicate effectively, both verbally and in writing
- Highly detail oriented with a critical degree of accuracy regarding data entry and analysis
- Working knowledge of Microsoft Office, including Word, Excel and Outlook is required
- Must have valid driver's license, auto liability insurance and reliable transportation if driving is required
- Successful completion of pre-employment physical, drug screen, 2-step TB test and background check
- Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects
- Physical activities: Climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, talking, hearing, repetitive motions, fingering (picking, pinching, typing, etc.), grasping (applying pressure with fingers and palm), feeling (perceiving size, shape, texture, temperature by touching), visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)
- Environment: Work is primarily performed in a climate-controlled environment with minimal safety/health hazard potential
- Time: Ability to be flexible with a varying schedule and programmatic requirements; Monday through Friday work week.
- Travel: May be required between locations
