HNYMPC is seeking Licensed Clinical Social Workers to focus on the assessment, diagnosis, treatment and prevention of behavioral conditions and management of stressful medical, social & emotional difficulties. HNYMPC primarily serves the geriatric patient population in a managed care environment. Our LCSW team focuses on providing individualized care plans and supportive, preventive tools and strategies to manage patient issues. The LCSWs participate in chronic disease management, including population-based education and address problems related to illness, treatment, and or life situations. Social determinants of health influence health outcomes, which social work plays a crucial role in addressing, to promote well-being. Our social work team focuses on factors like education, housing, food insecurity, transportation and access to healthcare, working to improve patients’ living conditions and overall health.
We are seeking LCSW Social Workers to join our team, with need for coverage in the Brooklyn and Queens areas. The ideal candidate for this position will coordinate the care of older patients in a variety of settings, including patient homes, PCP offices, hospitals, community health clinics, long-term and residential health care facilities, hospice settings, and outpatient/daytime health care centers. Telemedicine is also an integral part of services provided.
Responsibilities:
· Assess nature of patient situations by interviewing and reviewing personal history
· Assess current level of functioning, conditions and needs
· Develop and execute individual care plans and treatment strategies
· Determine appropriate milestones to gauge progress
· Communicate updates to all relevant parties as needed (Families and PCPs, etc.)
· Facilitate referrals to other healthcare professionals and community Programs/resources
· Maintain accurate documentation
· Performing administrative and record-keeping duties
Qualifications:
· Previous experience in social work, counseling, or other related fields
· Compassionate and caring demeanor
· Ability to build rapport with patients along with patient-centered practice

Pay: $80,000.00 - $92,000.00 per year
