Hospice & Home Health Community Liaison

Location: Las Vegas & Nye County, NV
Salary: $60,000–$70,000 per year (depending on experience) + Uncapped Commission

Overview:
Are you a passionate healthcare professional who excels at building relationships and making a difference? We're seeking a self-motivated and experienced Community Liaison to represent both Hospice and Home Health services throughout Las Vegas and Nye County.

Key Responsibilities:

• Promote and position hospice and home health services through outreach and relationship-building
• Develop and maintain referral partnerships with hospitals, SNFs, physicians, discharge planners, social workers, and community groups
• Conduct initial consultations and assist with consents for care
• Educate patients and referral sources on eligibility, including DOL/EEOICPA-related resources
• Represent the agency at community events, professional groups, and networking functions
• Collaborate with internal teams to ensure seamless communication and patient transitions
• Track referral activity and report on sales performance to meet growth goals

Qualifications:

• 2–3 years of healthcare sales or marketing experience
• Hospice and/or Home Health sales experience strongly preferred
• Bachelor’s degree preferred (experience may substitute)
• Knowledge of DOL/EEOICPA processes a plus
• Strong interpersonal, communication, and presentation skills
• Ability to work independently in a field-based role
• Must be fully vaccinated or have an approved exemption (CMS requirement)
• Valid driver's license and reliable transportation
• Willing to travel up to 50% within service areas

Schedule:

• Monday to Friday
• Occasional evenings and weekends as needed
• Flexibility with office reporting—primarily field-based
