Social Worker

Full-time Social Worker for our skilled nursing facility. The ideal candidate will have experience in long-term care/ sub-acute setting and must have previous healthcare experience.

Social Worker Duties:

• Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and Maintaining individualized care plans
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotion needs of the resident

Social Worker Requirements:

Master's Degree in Social Work

Long Term Care / Subacute Experience Required

Salary up to $70,000
