We are currently seeking a talented, compassionate Clinical Social Worker to provide care to patients & families residing in Charles County, Maryland. This position is Full-time with a schedule of 40-hours per week, Monday - Friday.

Want to join our Team? We are currently looking for dedicated Clinical Social Workers to join our team. As a Clinical Social Worker, you will provide psycho-social supports and interventions for those with life-limiting illnesses, helping patients and their caregivers navigate end-of-life care with compassion. You will work with your team’s RN Case Managers, Chaplains, and CNAs, as well as our Hospice Physicians, to provide physical, psychosocial, and spiritual aspects of care. We empower our Social Workers to take ownership of their day and schedule with an emphasis on care continuity and quality.

Office Location: Waldorf, MD

Coverage Area: Charles County

Status: Full-time

Salary: Competitive, commensurate with experience, education and credentialing

Main Responsibilities:

  • Assesses each patient/family admitted to Hospice of the Chesapeake (HOC) services for psychosocial issues impacting on the care-giving system, including an assessment of the environmental, age specific, spiritual and financial impact of loss on patients, families, caregivers, and clients.
  • Formulates individualized plans of care for patients based on assessed needs, strengths, limitations and goals. Includes patient and caregivers in the care planning process. Ensures that patient’s desires are taken into account when the treatment plan is formulated. Regularly reassesses for changing needs.
  • Participates actively in IDT meetings, functioning as an advocate for the patient and family. Communicates initial/ongoing plan of care appropriately to multidisciplinary treatment team.
  • Collaborates with providers and treatment team to identify and evaluate patient outcomes and to evaluate variances in expected behavior and appropriateness of care.
  • Maintains current referral information on local human services organizations and provides timely referrals to patient/caregivers as needed. Provides follow up as appropriate to assess effectiveness, and assumes an advocacy role, when indicated, to facilitate holistic care.
  • Provides counseling and emotional support and/or crisis intervention as per professional standards.

Your Qualifications

  • Master of Social Work degree from an accredited educational institution required. Minimum of LMSW licensing is required; LCSW-C preferred.
  • 2-3 years of practice in health care field, with experience in hospice, end-of-life, or critical care experience preferred.
  • Familiarity and comfort with documenting in an Electronic Medical Record.
  • Valid driver’s license and acceptable driving record and reliable transportation
  • Ability to meet the physical demands of the position with or without reasonable accommodation.
