Oversees social service designee

You'll Be Responsible For:

Overseeing the work of the Social Service Designee
Helping residents and families adjust and solve problems regarding their social, emotional, and financial needs
Assisting with Room and Roommate Changes
Attending Medicare Meetings
Discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements,
Advocating for residents and families regarding resident rights issues.
Conducting in-service training to educate staff about the emotional & social needs of residents, and offer methods for better serving residents and families
Attending administrative staff meetings, morning reports, Performance Improvement meetings, resident care-related meetings, and quarterly staff meetings
Maintaining departmental records and documentation as required
Coordinating and updating resident care conference schedule
Assisting residents and families with Medicaid approval and reapplication
Providing Medicaid Pending and Patient Liability related follow-up

You'll Need To Have:

A Bachelors Degree in Social Work
Active Licensed Social Worker Certification in the state where facility is located (OH)
Long Term Care Experience or willingness to learn
Excellent communication and listening skills
The ability to identify residents' social, emotional, and financial needs
