Arisa Health is seeking candidates to fill a Care Coordinator position in Harrison, AR. The Care Coordinator is a member of the health care team who helps individuals overcome possible barriers to care. The Care Coordinator works in collaboration with clients, multidisciplinary care providers and community partners to coordinate treatment and services. The person who fills this position will provide timely intervention and follow up to individuals experiencing a behavioral health crisis. They will develop care initiatives to assess and coordinate care management for clients and serve as a client advocate and will assist in the identification and improvement of service delivery.

Arisa provides outpatient services, Crisis services and emergency screening, substance abuse counseling, intensive children services, intensive adult services, medical services, and residential services in 41 Arkansas counties.

This position is classified as full-time, non-exempt (hourly).
Work hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.

What our Care Coordinator will do:

  • Provide individual client/family education and self-management support that is appropriate based on language, cognitive abilities, literacy level, learning style, cultural norms, patient preference, readiness for change and resources available.
  • Assist in facilitating transition and provide care coordination for those being discharged from inpatient settings such as hospitals and rehabilitation facilities, as well as those seen in emergency departments and urgent care facilities. Will follow up with clients within 1 day of discharge from facility.
  • Address medication and treatment adherence.
  • Participate in multidisciplinary staffing(s), team meetings, staff meetings and quality improvement projects to improve client care.
  • Communicate changes in client’s status and document all communication with and/or on behalf of client in electronic medical record.
  • Identify clients who are overdue for visits, labs, and referrals, contact clients and arrange for follow-up services, and follow-up with clients to ensure services are received. May include arranging transportation or transporting client to and from appointments.
  • Consult with behavioral health and primary care providers including medical staff, nursing staff and ancillary department staff to eliminate barriers to the efficient delivery and access to care.
  • Serve as clinical resource and community referral resource and assist clients with identifying and accessing community resources.

What we look for in a Care Coordinator:

  • Bachelor’s degree in human services, Behavioral Services, or related field is required
  • Candidates must be proficient with computers including the ability to type, and familiarity with word processing and e-mail programs.
  • Good phone etiquette required.
  • Excellent written and verbal communication skills.
  • Ability to handle multiple tasks.
  • Proven ability to work independently with minimum supervision.
  • Familiarity with the behavioral and primary health care system(s).
  • Knowledge of or the ability to learn local community resources.
  • Ability to establish and maintain effective working relationships with others.
