Nexus-PATH is looking for a full-time Care Coordinator to work with our youth clients at our Luther Hall residential location in Fargo, ND!

Our Care Coordinator is responsible for providing case management to support the delivery of effective and efficient care in a 24-7 youth & teen residential care in a Psychiatric Residential Treatment Facility (PRTF) working to create a healthy environment to bring about positive changes in behaviors, attitudes, and values of youth residing in the facility.

Schedule/Location/Pay:

  • Monday-Friday: 8:30am-5:00pm
  • 100% On-site: 1505 5th Avenue South, Fargo, ND 58103
  • Pay Rate: $23+ per hour

Responsibilities

  • Provide mental health and counseling services to assigned youth, ensuring service provision meets standards
  • Manage and coordinate resources and activities to meet clients' unique treatment needs
  • Facilitate case conferences and team meetings with internal and external resources
  • Implement individual treatment plans, compile assessment materials, and maintain clinical documentation
  • Collaborate with family members, licensing authorities, parole officers, and the court system to ensure consensus on treatment direction and outcomes
  • Maintain individual accounts and residential records, ensuring clients' basic needs are met
  • Utilize organizational skills to manage multiple client needs and represent the organization positively
  • Maintain a flexible schedule to meet changing work needs and demands
  • Facilitate client and family linkage to the therapeutic treatment process and support post-transition placement
  • Develop treatment plans and therapeutic interventions with interdisciplinary teams
  • Assist families in locating resources to support healthy family functioning
  • Bridge relationships with clients and outside organizations, advocating for needs
  • Participate in required meetings and conferences related to assigned clients
  • Exhibit professional and effective communication with clients, coworkers, families, and external contacts
  • Maintain and complete required paperwork and documentation
  • Provide consultation and mental health services, documenting services consistent with Medicaid requirements
  • Ensure completion of required documentation standards, including incident reporting
  • Communicate significant events to supervisory staff to ensure continuity of care and client safety
  • Participate in training and development of care coordinators and assume mentorship roles.
  • Perform First Aid/CPR procedures and respond to emergencies according to protocols
  • Maintain appropriate boundaries in interactions with residents
  • Comply with privacy and confidentiality policies for residents and their families
  • Handle emergency or crisis situations with sound judgment and appropriate actions
  • Manage stressful workload conditions and emotionally charged situations effectively
  • Demonstrate concern and empathy while maintaining boundaries
  • Exhibit ethical conduct, integrity, and confidentiality, adhering to the Nexus Code of Conduct
  • Demonstrate self-awareness and professional use of self in day-to-day work situations
  • Recognize and value cultural differences in all aspects of work and service delivery
  • Demonstrate professionalism, work accountability, and cooperation among team members

Required Qualifications

  • BA degree in Psychology, Social Work, Education, Criminal justice, Child development, or other behavioral field required
  • Minimum 21 years of age for all clinical program serving clients older than 12 years of age
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
