Responsibilities:

QUALIFICATIONS:

Graduate of accredited LPN program or bachelor’s degree in human service field, such as Social Work, Psychology, Rehabilitative Counseling, or closely related discipline preferred.
Five or more years of behavioral health direct care experience can be substituted for Bachelor’s degree.
Computer literacy including past experience with keyboarding, email, internet and computerized health record systems is desired.
Preference will be given to candidates that may meet the cultural and linguistic needs of the geographic needs and area.
Multilingual candidates are preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The Care Coordinator works closely with treatment team to collaborate aftercare.
Acts as liaison between the agency, enrollee and outpatient providers, community resources and supports.
Integrates the PRISMModel® for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.
Assist client in assessing appropriate resources, programs and supports including completion of necessary paperwork. Keep this
Document in an electronic health record member’s information, activities, care plans and services. Complete a Comprehensive Assessment and Care Plan for every engaged member. Work with Primary Care Providers on Care Plan approval.
