QUALIFICATIONS:
- Graduate of accredited LPN program or bachelor’s degree in human service field, such as Social Work, Psychology, Rehabilitative Counseling, or closely related discipline preferred.
- Five or more years of behavioral health direct care experience can be substituted for Bachelor’s degree.
- Computer literacy including past experience with keyboarding, email, internet and computerized health record systems is desired.
- Preference will be given to candidates that may meet the cultural and linguistic needs of the geographic needs and area.
- Multilingual candidates are preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- The Care Coordinator works closely with treatment team to collaborate aftercare.
- Acts as liaison between the agency, enrollee and outpatient providers, community resources and supports.
- Integrates the PRISMModel® for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.
- Assist client in assessing appropriate resources, programs and supports including completion of necessary paperwork. Keep this
- Document in an electronic health record member’s information, activities, care plans and services. Complete a Comprehensive Assessment and Care Plan for every engaged member. Work with Primary Care Providers on Care Plan approval.
