Overview

Working as part of a scheduling team or individually, this position will be responsible for coordinating and maintaining schedules for FirstLight Home Care clients and caregivers, building caregiver relationships, completing in-home client assessments, new caregiver introductions, as well as assisting with client visit coverage when callouts occur. This position will directly interface with clients on multiple levels.

Duties

Essential job duties for this position include the following items. Other duties must be performed as assigned or required.

▪ Schedule shifts by matching caregiver availability and qualifications with clients’ needs

▪ Conduct client intakes and develop Care Plans

▪ Answer phones, and contact clients /families and caregivers for scheduling updates and relationship building

▪ Assist with initial and ongoing in-home evaluations, caregiver introductions, PPE delivery, and coordination of client services

▪ Assist with day-to-day office and field duties to ensure quality assurance of services

▪ Oversee and coach caregivers, and provide Director with vital feedback for caregiver performance appraisals

▪ Review/Complete appropriate visit records in a timely manner and according to policy

▪ Take on-call responsibilities to answer and act upon calls after hours and weekends to handle emergencies and scheduling issues.

▪ Stay current with changing technology, including software programs.

▪ Uphold, support, and promote all company policies and procedures.

▪ Adhere to all HIPAA privacy regulations

QUALIFICATIONS:

To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, abilities, certifications, and educational experience required.

1. EDUCATION/EXPERIENCE

▪ Associate degree (A.A.) or equivalent from two-year college or technical school, or 2 years related health care/home care industry experience and/or training preferred; or equivalent combination of education and experience

▪ At least one year of experience with scheduling in a healthcare setting

▪ Ability to effectively interact with clients, families, and FirstLight team.

▪ Experience as a HHA/CNA/PCA and skilled in providing care to clients with various levels of care, as needed, preferred.

▪ Computer and Internet fluency, proficiency with Microsoft Office and/or Google Suite and scheduling software required

2. CERTIFICATIONS, LICENSURE & REGISTRATION

▪ Possess and maintain a valid Driver’s License and maintain adequate auto insurance; travel required

3. LANGUAGE SKILLS

▪ Ability to read and understand short instructions, correspondence, and memos

▪ Ability to effectively communicate through written and verbal language

▪ Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization

4. REASONING ABILITIES

▪ Ability to evaluate situations and make complex decisions using critical thinking and reasoning

▪ Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

▪ Ability to discern when to escalate client situations, such as calling Supervisor or 911

5. OTHER SKILLS & ABILITIES

▪ Must possess and demonstrate excellent organizational, communication, interpersonal and team building skills as well as maintain a positive and professional image to work effectively with multi-cultural caregivers

▪ Ability to multi-task and work in a fast-paced environment with changing priorities

▪ Self-directed individual with strong prioritization and time management skills

▪ Collaborative team player

▪ Must undergo and successfully pass a background check and drug test
