Job Title: RN Care Manager

Job Summary:
We are seeking an experienced and dedicated Registered Nurse (RN) Care Manager to join our Case Management team, specializing in the coordination of care for high-risk patients. The RN Care Manager plays a critical role in assessing, planning, implementing, and evaluating care in collaboration with an interdisciplinary team. This position ensures that patients with complex medical and psychosocial needs receive high-quality, cost-effective care with seamless transitions across the continuum. The ideal candidate is a strong clinical decision-maker, a patient advocate, and a proactive problem solver with experience in both acute and post-acute care settings.
Key Responsibilities:
- Serve as a primary RN Care Manager for high-risk patients, focusing on complex length of stay issues, readmissions, and frequent ED utilizers.
- Conduct comprehensive nursing assessments to identify medical, psychosocial, and discharge needs.
- Collaborate with social workers, physicians, and the care team to develop individualized care plans that promote optimal health outcomes and safe transitions of care.
- Regularly review and revise plans of care to align with patient progress and goals.
- Ensure compliance with utilization guidelines, payer requirements, and federal/state regulations in all care transitions.
- Communicate clearly and consistently with patients, families, providers, and external partners to support a smooth care experience.
- Advocate for patient access to care by identifying and addressing barriers within the healthcare system and the community.
- Maintain up-to-date knowledge of health insurance, community resources, and case management best practices.
- Educate patients, families, and hospital staff on care plans, services, and resource options.
- Build and maintain strong professional relationships with internal and external stakeholders, including community resources and referral sources.
Qualifications:
- Bachelor’s degree in Nursing (BSN) required.
- Active RN license in the state of Connecticut required.
- Minimum of 3 years of clinical experience in an acute care setting.
- Experience with high-risk patient populations, home care, long-term care, or case management preferred.
- Case Management Certification (CCM or ACM) strongly preferred.
- Excellent critical thinking, communication, and organizational skills.
- Knowledge of insurance regulations, discharge planning, and care coordination across settings.
Preferred Skills:
- Prior case management or utilization review experience.
- Ability to manage diverse and complex patient populations across the care continuum.
- Familiarity with EMR systems and strong computer skills.
Work Environment:
