Geriatric Mental Health Therapist
The Program Therapist is responsible for a variety of patient care services; with the primary focus being the emotional, behavioral, and mental well-being of patients served. Under the direction and supervision of the Medical Director/Assistant Medical Director; the Program Therapist will conduct assessments and provide individual, group, and family therapy. In collaboration with the Treatment Team the Therapist will formulate the plan of treatment and update as needed. In addition to these services the Program Therapist will serve as Case Manager to coordinate discharge planning, ensuring communication with caregivers/family members, maintaining contact with referral sources, and other service providers.
Duties and Responsibilities
Actively supports the mission, philosophy, and goals of Lake Behavioral Health and the host hospital.
Utilizes appropriate techniques for the evaluation and treatment of patients.
Interviews patients/families/caregivers to obtain history and development of the treatment plan.
Provides individual, group, and family therapy as ordered by the Medical Director/Assistant Medical Director.
Communicates with the patient and caregivers in identifying treatment goals and progress.
Consults with other disciplines concerning treatment plan, patient and referral needs.
Maintains regular contact with facilities, referral sources, agencies, or other professionals associated with the patient.
Actively participates in treatment team, treatment plan development, and revisions.
Develops and implements appropriate discharge planning with patient, caregivers, agencies, and/or facilities.
Documents all clinical and case management services to meet CMS and other accreditation regulatory standards.
Assists in all paperwork requirements from pre-admission to post discharge.
Complies with all administrative, clinical, human resource, regulatory agencies, state and local regulations, and host hospital standards.
Adheres to work practices consistent with Lake Behavioral Health and the host hospital mission, medical staff bylaws, administrative, departmental, and human resources policies and procedures.
Maintains the highest level of ethics in all program activities.
Assist in referral development and participation in networking opportunities.
Participates and complies with the program’s performance improvement and quality measures.
Fulfills other duties as assigned.
Qualifications
Master’s degree in social work, psychology, counseling, or a comparable behavioral healthcare degree. Must be licensed to practice respective discipline independently. Consideration may be given to one who requires supervision. Experience working with program’s population and behavioral health care facility preferred.
CLINICAL SUPERVISION FOR LICENSURE IS PROVIDED
Job Type: Full-time
Pay: $55,000.00 - $64,000.00 per year
