Caring Hearts of Ohio seeks a compassionate and skilled Clinical Therapist to provide high-quality, trauma-informed therapeutic care and case management services to teen boys ages 12-18 residing at our youth group home. This role involves delivering individual, family, and group therapies, as well as managing cases to ensure continuity of care. The ideal candidate will collaborate with multidisciplinary teams to develop and implement individualized treatment plans, participate in on-call crisis management, and foster a trauma-responsive organizational culture.
Responsibilities:
- Therapeutic Services:
- Provide individual, family, and group therapy to teen boys and their families.
- Conduct assessments to make treatment and level-of-care recommendations based on diagnosis and medical necessity.
- Develop and implement individualized service plans in collaboration with residents and families.
- Participate in on-call rotation for crisis intervention, including in-person and telehealth services as appropriate.
- Case Management:
- Coordinate services between residents, families, and external providers to ensure continuity of care.
- Assist residents and families with accessing resources such as educational support, vocational training, and mental health services.
- Collaborate with referring agencies and other service providers to maintain seamless communication and coordination of care.
- Monitor residents’ progress and adjust service plans as necessary to meet evolving needs.
- Serve as an advocate for residents during multidisciplinary team meetings and discharge planning.
- Compliance and Documentation:
- Maintain accurate, timely documentation of therapeutic and case management services, progress notes, and treatment goals.
- Ensure compliance with all relevant standards, including HIPAA, ODJFS-QRTP, and OhioMHAS guidelines.
- Trauma-Informed Culture:
- Contribute to a trauma-informed and responsive organizational ethos by modeling therapeutic interventions and teaching coping strategies.
- Support safe de-escalation and stabilization of residents during times of crisis.
- Professional Development:
- Actively participate in supervision, case reviews, and ongoing training to enhance clinical and case management skills.
