What We’re Looking For
Trillium Health Resources has a career opening for an RCT Regional Care Clinician to join the Regional Care Team! The RCT Regional Care Clinician is responsible for ensuring assigned members receive whole person-centered care with foundations in national evidence based and informed standards to meet their physical health, mental health, and social needs. This position serves as the single point of contact for members and works with a multidisciplinary care team, including primary care providers, specialists, care manager extenders, transition staff, and caretakers/legal guardians, to promote continuity of care and quality care for each member. On a typical day, you might:
- Serve as the primary point of contact for the member, provider, and natural support/guardian for all Tailored Care Management (TCM) activities involving the member.
- Provide oversight and/or outreach, engagement, and support of members receiving Tailored Care Management.
- Complete Care Management Comprehensive Assessments of members’ needs and utilizes the responses to formulate clinically based conclusions on member needs and to guide the development of an individualized care plan, including prioritized treatment goals, treatment providers and services.
- Ensure the initiation and continuance of integrated care plans that incorporate a member’s physical health, mental health, social supports lifestyle, community involvement, and potential health risks.
- Facilitate regular meetings with members, in-person, audio-visually and/or telephonically.
- Consult with providers and others of the multidisciplinary team to gather and share information relevant to each member’s care including care management comprehensive assessments and reassessments.
- Ensure medication management is conducted by the most appropriate team member, and utilizes the results of medication management, including medication reconciliation and medication adherence as part of the integrated care plan.

Qualifications:
A Bachelor’s degree as a licensed RN with one (1) year of experience providing care management, case management, or care coordination to the MH/SU/BH population.
Preferred Education:
Preferred degree in a field related to Health, Psychology, Sociology, Social Work, Nursing, or any other relevant Human Services field.
Fully licensed in the State of North Carolina as a LCSW, LCMHC, LPA, LMFT, LCAS or RN.
Must have a valid driver’s license.
Location:
Must reside in NC to be considered for remote status.
Must be able to travel to a Trillium office location and within catchment as required.
