We are looking for a Care Transition Coordinator to join our team. This position will directly report to the VP Care Coordination and is responsible for supporting the Care transitions team in the navigation of patients between the Gentiva care setting. This role is under established performance criteria.
- Responsible for taking/performing chart reviews and supporting referral coordination for patients
- Adheres to and participates in Company’s mandatory training which includes but is not limited to HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and Company policies and procedures
- Document and track patients within a Gentiva system, and send referrals as needed per Director of Care transitions directives
- Reviews medical records against nationally recognized clinical criteria guidelines
- This is a work from home telephonic position
The shift is Monday- Friday 8-4:30 EST.
About You:- Minimum of 1 year working in a healthcare related field or healthcare call center
- INTERNET REQUIREMENT: High-speed Internet connection (minimum 10 Mb/s download speed and minimum 2 Mb/s upload speed, recommended 5 Mb/s upload). Satellite based internet services are not acceptable
- Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills
