Care Manager RN – Specialty Pharmacy Highlights:

• Schedule: 9:30 AM – 6:00 PM EST (EST Time Zone Preferred)
• Work Location: Fully Remote – U.S. only
• Pay Range: $42-48 Hourly

Care Manager RN – Specialty Pharmacy Responsibilities:

• Manage assigned and pended worklists
• Conduct inbound/outbound patient calls for clinical intervention
• Collaborate with internal departments and physician offices
• Respond to emails and internal messages promptly
• Attend trainings and meetings regularly
• Maintain high-quality performance and meet department KPIs
• Evaluate member needs and social determinants of health
• Develop, implement, and monitor personalized care/service plans
• Identify and address barriers to care, coordinating with caregivers and providers
• Educate members on disease processes and healthcare options
• Ensure documentation meets regulatory and compliance standards
• Participate in outreach activities via phone, digital visits
• Support care improvements and provide feedback

Care Manager RN – Specialty Pharmacy Qualifications:

• Active RN license (compact license required)
• 2+ years of clinical experience in acute, community, or clinical setting
• 1+ years of case management experience in managed care
• Experience in specialty pharmacy
• Familiarity with utilization management, InterQual, and NCCN guidelines
• Strong knowledge of managed care and decision support tools
• Preferred: Associate’s or Bachelor’s Degree in Nursing (BSN)
