As a Service Coordinator, you'll play a key role in connecting our residents - many of whom are elderly or living with disabilities - to the resources, programs, and services that support independent, healthy, and fulfilling lives.
Requirements
What You'll Do:
- Serve as the primary link between residents and community-based resources - everything from transportation and financial services to health programs and social activities.
- Coordinate individualized care plans to help residents achieve the highest level of well-being - physically, mentally, and socially.
- Create and oversee on-site programs that reflect current social service trends and residents' evolving needs.
- Build meaningful relationships with residents and act as a trusted guide and advocate.
- Collaborate with community organizations and attend events to promote services and build partnerships.
- Monitor social services budgets and assist with program reporting and compliance.
- Full-time hours: Monday through Friday, 8:00 AM - 5:00 PM with an hour lunch

- Strong communication skills and a compassionate, professional demeanor
