Job Summary

The Geriatric Care Manager (CM) is responsible for managing and providing Care Management services to clients.

Care management services include meeting with prospective clients and providing a professional assessment and plan of care. The CM may provide ongoing monitoring to ensure quality, appropriateness and cost effective services to clients. CM uses specialized knowledge and regularly exercises discretion and independent judgment and makes decisions on a case by case basis. CM will participate in the ongoing assessment, diagnosis and treatment of case management clients. CM will work with other professionals involved in client care with the assessment, diagnosis and treatment of clients.

Responsibilities and Duties

  • Care Management
  • Asses clients on a weekly or monthly basis
  • Make appropriate diagnosis of medical changes or concerns and notify appropriate professionals involved.
  • Participate in the treatment of psychiatric diagnosis and care plan.
  • Work with a geriatric population and special needs clients, their families and caregivers to provide case management services with the goal of supporting independence, safety and preventing premature institutionalization.
  • Conduct comprehensive assessment to determine client needs and assist in development of plan of care. Participate in the diagnosis, treatment and assessment of clients.
  • Serve as a liaison between medical, psychiatric, social service and community agency providers and individual clients.
  • Monitor quality and effectiveness of services provided to client making changes to care plan as needed.
  • Document contacts with client, family, providers and ancillary services.
  • Keep running log of all services delivered for billing purposes.
  • Maintain knowledge of and working relationship with community resources.
  • May promote SSA and general eldercare services and education to the community via special events, conference and community presentations.
  • Provide consultation, counseling and education to clients, caregivers and family members and other agency personnel.
  • Assures the provision of safe, high-quality services to clients.
  • Ensures the execution of agency required client service agreement documentation along with an explanation of services provided.
  • Provides after hours and weekend advice as needed to the after-hours coordinator
  • Performs service coordination when necessary during afterhours and weekends
  • During an emergency contacts manager for assignments which may be out of daily responsibilities.
  • Performs other duties as assigned consistent with the skills and training and the goals and mission of SSA.
  • As needed: assists clients, family members, caregivers, and other agency representatives in understanding the significant social, emotional and environmental factors which might impact a person’s ability to maintain an independent living status.
  • Participates in the development of the plan of care. Prepares clinical assessments and progress notes as appropriate according to accepted Case Management Standards. Works with the family, caregivers and/or significant others.
  • Provides referral to and utilization of appropriate community resources.
  • Attends staff meetings; in-service agency meetings and educational community meetings.
  • Screening, arranging, and monitoring in-home help.
  • Acting as a liaison to families at a distance.
  • Assisting with moving their clients to or from different care settings.
  • Reviewing financial, legal, or medical issues and referring clients to experts.
  • Providing crisis intervention.
  • Providing client and family education.
  • Visiting clients on a regular, routine basis to make sure they are safe, doing well, eating properly, and taking needed medications.
  • Making necessary medical appointments and the assuring client gets to them.

Qualifications and Skills

  • Registered nurse (RN) licensed in Texas or in accordance with the Texas Board of Nursing.
  • At least one-year experience as an RN in a clinical role
  • Knowledge of local resources
  • Proof of a current driver's license, insurance and dependable automobile.
