Key Responsibilities:

  • Coordinate individualized care plans for clients in need of support services
  • Provide addiction counseling and crisis intervention when appropriate
  • Mentor and guide clients toward achieving personal and professional goals
  • Use motivational interviewing techniques to inspire positive change
  • Respond to crises while ensuring the safety and well-being of all clients
  • Connect clients with community resources, including:
  • State/County benefits
  • Employment and housing assistance
  • Healthcare and substance use services
  • Assist with workforce development and job readiness for clients
  • Conduct intake screenings and manage discharge planning
  • Manage a caseload of 20+ individuals with 75% field-based outreach

Qualifications:

  • Minimum 2 years of recent experience in case management or a related field
  • Experience with Transition Aged Youth (TAY) strongly preferred
  • Bachelor's degree in Psychology, Social Work, Counseling, or related field preferred
  • (HS Diploma + 2 years of experience may be substituted)
  • Strong interpersonal and crisis management skills
  • Must be able to work in the field 75% of the time
  • Valid CA driver’s license, auto insurance, and insurable driving record
  • Certification or training in addiction counseling is a plus

Job Type: Full-time

Pay: $29.00 per hour

Expected hours: 40 per week
