Seeking Registered Nurse Care Coordinator to launch a new standard of senior home care services in Keller / N. Fort Worth,TX. Job Summary: Ensure optimal client care by serving as their primary point of contact for questions, concerns, and feedback, as well as serve as the primary RN involved in the creation and maintenance of the care plan.

Qualifications/Educational Requirements:

1. Be a registered nurse (R.N.).

2. Minimum of two years of nursing experience.

3. Ability to relate positively and favorably with clients and staff.

4. Demonstrate good oral and written communication along with good documentation skills.

Responsibilities / Essential job functions:

1. Communicate with the client and the client’s representatives via all communication channels, including the Family Portal, regarding any questions, concerns, comments, or changes that are requested.

2. Communicate with the scheduling department on behalf of the client when additions, subtractions, or alterations need to be made to the client’s schedule of care.

3. Communicate with the caregivers regarding any information that is deemed pertinent for client care. This should include contacting every caregiver prior to their initial shift with a client to ensure that they’ve read the plan of care and understand the client’s needs.

4. Coordinate care for the patient amongst all parties involved.

5. Document any and all events relating to client and/or caregiver in ADLware.

6. Conduct the 90-day care plan and nursing reassessments on time and without prompting.

7. Supervise the CNA / GNA staff as required.

8. Observe, assess, and document patient symptom and progress.

9. Notify HIPPA representative of change in the patient’s condition.

10. Coordinate and monitor all patient care and services.

11. Complete paperwork timely and efficiently.

12. Maintain patient records according to policy and procedure.

13. Follow infection control policy in and out of the office.

14. Provide quality of care to patients in their homes according to policy and procedures.

15. Participate in and support quality improvement programs.

16. Prepare and conduct new CNA/GNA orientation courses (as needed)

17. Participate in the ongoing review and refinement of CNA/GNA instruction and review protocol
