Responsibilities of a Social Worker:

• Conduct assessments to identify the needs of residents and develop individualized care plans.
• Provide counseling and support to residents and their families.
• Advocate for residents’ rights and ensure their voices are heard.
• Coordinate with healthcare providers, community resources, and family members.
• Facilitate support groups and educational sessions for residents and families.
• Assist with discharge planning and follow-up services.

Qualifications of a Social Worker:

• RNs with a background in social services or psychology are welcome to apply!
