Position Title: Social Worker


Location:
Greenville, OH, US


Employment Status:
Full-time role requiring 40 hours per week


Schedule:
Monday to Friday, 8:00 am to 4:30 pm, with on-call shifts during evenings, weekends, and holidays


Responsibilities:
Managing and executing psycho-social programs customized for patients and their families based on personalized care plans

  • Conducting thorough initial and ongoing psychosocial assessments for each patient and their family, or upon request, adhering to accepted professional standards and agency guidelines.
  • Evaluating patients and family members for potential risks related to abuse, neglect, and suicide, as well as those with current or past substance abuse issues, and establishing appropriate agreements when necessary.
  • Delivering effective counseling methods and procedures for patients and families, encompassing therapeutic and supportive interventions, education, life review, crisis intervention, and suicide prevention when indicated.
  • Supporting patients and families with end-of-life care planning, including providing information on living wills, durable power of attorney (DPOA), care provisions for dependents, assessing decision-making capacity, and funeral arrangements.
  • Developing and implementing a patient/family plan of care and participating in Interdisciplinary Group (IDG) and care conferences as required.
  • Leveraging available community resources to support patients and their families.
  • Helping members of the interdisciplinary team understand the significant social and emotional aspects connected to the patient's healthcare journey.
  • Coordinating visits with other team members to ensure patient and family needs are met most effectively.
  • Ensuring seamless care transitions when a change in the care setting is necessary (such as admission to a nursing home or respite care). This includes facilitating nursing facility placement (PAS/RRs) and arranging transportation.
  • Educating employees, volunteers, and the community about available services that address the psychosocial and economic needs of patients.
  • Providing assistance to the Bereavement Coordinator as requested.
  • Working evenings and weekends as needed to accommodate the needs of patients and their families.
  • Completing all required documentation in accordance with Agency policy, accreditation standards, and regulatory requirements.
  • Performing other assigned duties.
Qualifications:

Skills and Qualifications:

  • To be considered for the Social Worker opening in Greenville, Ohio, United States, you'll need a Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE)-accredited program. We may also consider candidates with a Bachelor of Social Work (BSW) degree.
  • Holding a current Social Work license in the State of Ohio is a must.
  • We're looking for individuals with at least two years of experience in a clinical environment.
  • Prior experience within healthcare and/or hospice settings would be a plus.
  • A valid driver's license is required for this position.
  • Exceptional customer service abilities are key.
  • The ideal candidate will be detail-oriented and possess strong skills in communication, documentation, and organization.
  • It's important to be able to follow instructions precisely.
  • We need someone who can work autonomously without constant direct supervision.
  • Flexibility to travel to other locations as necessary, even when the weather is bad, is expected.
  • You should be able to lift up to 25 pounds of supplies or equipment.
  • The position involves a range of physical activities, including sitting, standing, twisting, turning, bending, lifting, and walking.
